Observation

The Center does not have one document labeled "Individual Care Plan" for children with life threatening allergies. Instead, they have multiple documents, stored in multiple locations, that meet the requirements of WAC 110-300-0186(1) and (3). These multiple documents were complete for the child named in the referral. The cook and the staff person relieving the lead teacher for her lunch break did not review all elements of the child's Individual Care Plan prior to preparing, or serving, the child food. The child was fed Ranch dressing containing eggs. The child is severely allergic to eggs.
